State Cup Online Application

Please click on the State Cup Age Group that you would like to apply for Under 11 through Under 13 State Cup is now CLOSED!!!   Under 14 through Under 18 State Cup is now CLOSED!!! If you wish to apply for the Reserve Pool you can submit your application ONLY by a paper application.  You can mail or fax your application with the appropriate fees.  Only MasterCard, Visa, Cashiers Check or Money Order will be accepted.  NO Personal Checks!!!   State Cup Paper Application   Under 19 Please remember the following deadlines:   Under 11 though Under 18: Applications MUST be received by the CYSA State Office NO later than November 1st, 2005 by 6:00 PM   Under 19: Applications Must be received by the CYSA State Office NO later than April 3rd, 2006 by 6:00 PM   If you choose not to apply online you can still fill out the paper application and send it to the CYSA State Office   State Cup Paper Application To Return to your Online Application (You will need your User ID and Password to get back to your Online Application)   Under 11 through Under 13   Under 14 through Under 18   Under 19 To check if your State Cup Application has been received click here State Cup General Information
