League Affiliation Important Notice

Note: League Affiliation Agreement form MUST be signed by two League Officials and submitted with a check for $100.00 payable to CYSA.

  1. A copy of the Leagues most current and up to date Operating Procedures (Constitution, Bylaws, General Rules).

  2. Leagues MUST also submit proof of current "Good Standing" with the Internal Revenue Service (IRS) and Secretary of State.

Form 8201 2005/2006 League Affiliation Agreement

This agreement and the affiliation represented by it shall become effective only when signed, and approved by, California Youth

 Soccer Association, Inc. State Board of Directors.

 

By filling out this form and submitting your payment does not guarantee League Affiliation with California Youth Soccer Association until

 the CYSA State Board of Directors has signed and approved affiliation

All New Leagues MUST submit proof of their Non-Profit status and a copy of their approved Articles of Incorporation.

Returning Leagues are only required to submit proof of Current "Good Standing" with the Internal Revenue Service and the

State of California (for fiscal non-profit status and Incorporation), respectively, every odd numbered year of application for

affiliation with the California Youth Soccer Association, Inc.

bulletOperating Procedures (League Constitution, Bylaws, General Rules
bulletNew Leagues MUST submit: Proof of their Non-Profit status and copy of their approved Articles of Incorporation
bulletDistrict Commissioners MUST submit Form 8201 2005/2006 League Affiliation Agreement by Tuesday, March 1st, 2005 to the CYSA State Office

Incomplete or missing 2005/2006 League Affiliation Agreement materials such as Operating Procedures (League Constitution, Bylaws, General Rules), proof of Non-Profit status, Good Standing with Internal Revenue Service and Good Standing with State of California will be returned to

League and considered as not having ever been submitted. If materials are missing, will not guarantee affiliation for the 2005/2006 season and

subject to fine.

All member leagues, their teams and players, shall abide by the Constitution and Bylaws of the California Youth Soccer Association, inc., the United States

Youth Soccer Association, and the United States Soccer Federation; All General Procedures and Specific Rules are set forth by the Board of Directors and all application rules and regulations of the United States Youth Soccer Association and the United States Soccer Federation.
